Embryo-Implantation III: Rezeptivität des Endometriums

Überblick

Schlaglichter:

  • Die Einnistung des Embryos im Uterus ist ein biologisches Phänomen von großer Faszination: Sie ist nicht nur von ausschlaggebender Bedeutung für die Etablierung der Schwangerschaft sondern auch ein Modellfall für die Interaktion von invasiven Zellen (hier: des Trophoblasten der Blastozyste) mit ihrem Wirtsgewebe (hier: dem Endometrium des Uterus), und es ergibt sich die Frage, ob dies Parallelen zur Tumorzellinvasion aufweisen könnte.
  • Die Embryo-Implantation kann nur in Gang kommen, wenn simultan der Trophoblast der Blastozyste und das Partnergewebe, das Endometrium, einen besonderen zellbiologischen Zustand erreicht haben: Invasivität im Fall des Trophoblasten, Rezeptivität im Fall des Endometriums.
  • Serin-Proteinasen, die wir im Themenbereich (4) bearbeitet haben, sind von zentraler Bedeutung für die Auflösung der Blastozystenhüllen, nicht aber für die nachfolgenden Phasen der Implantation, d.h. die Anheftung des Trophoblasten und die Penetration durch das Uterusepithel. Dies haben unsere Proteinase-Inhibitionsexperimente in vivo gezeigt. Für das Verständnis der Interaktion des Trophoblasten mit dem Uterusepithel müssen zellbiologische Erklärungen herangezogen werden.
  • Die Zellbiologie der Rezeptivität des Endometriums (wie auch die der Invasivität des Trophoblasten) ist weitgehend unverstanden geblieben. Um hier voranzukommen war es hilfreich zunächst zu erkennen, dass wir es mit einem zellbiologischen Paradox zu tun haben: Trophoblast und Uterusepithel etablieren bei der Implantation ihren ersten zellulären Kontakt vermittels ihrer apikalen Zellpole, obwohl einschichtige Epithelien eigentlich dadurch gekennzeichnet sind, dass ihre apikalen Zellpole nicht-adhäsiv sind. Wir haben für das Phänomen ein Erklärungsmodell entwickelt; es postuliert, dass die Grundprozesse Parallelen aufweisen zu einem anderen biologischen Phänomen, den epithelialen Fusionsprozessen in der Embryonalentwicklung. Bei diesen (und ebenso bei der Tumorzell-Invasion) spielt der Vorgang der sogenannten Epithel-Mesenchym-Transition (EMT) eine elementare Rolle. Wir haben postuliert, dass daher die Übertragung von Erkenntnissen über EMT-Vorgänge auf die Erforschung der Zellbiologie der Embryo-Implantation (und umgekehrt) von heuristischem Wert sein muss.
  • Die detaillierte zellbiologische Analyse zeigt, dass beim Eintritt des Uterusepithels in den rezeptiven Zustand Änderungen in einem wesentlichen Parameter dieser Zellen, nämlich der apiko-basalen Polarität, von zentraler Bedeutung sind. Der Verlust der (Epithel-typischen) apiko-basalen Polarität ist ein wesentliches Element der EMT. Das Uterusepithel vollzieht bei Eintritt in den rezeptiven Zustand und während der Trophoblast-Invasion aber keine komplette EMT, sondern es werden EMT-Elemente an strategisch entscheidender Stelle eingesetzt: partieller Verlust bzw. Destabilisierung dieser Polarität.
  • Wir haben in-vitro-Systeme entwickelt, die es gestatten, diese Phänomene an menschlichen Zellen (Zell-Linien) zu untersuchen, ohne menschliche Embryonen opfern zu müssen: Adhäsions- und Invasions-Modelle unter Verwendung von Multizell-Sphäroiden aus Trophoblastzellen und von endometrialen Zell-Linien oder Endometrium-Explantaten. Eine Verwendung menschlicher Embryonen (z.B. überzähliger Embryonen aus in-vitro-Fertilisations-Programmen) für die Forschung halten wir dagegen für ethisch unvertretbar.
  • Wir haben erstmalig die Adhäsionskräfte gemessen, die sich zwischen lebenden Zellen während der Kontaktaufnahme ausbilden, und diese über die Zeit verfolgt; Untersuchung von Signalübertragungsmechanismen und der Rolle des Zytoskeletts; pharmakologische Beeinflussung des Adhäsionsvorgangs; Rolle verschiedener Adhäsionsproteine, Glykokonjugate und Lektine.
  • Diese Untersuchungen haben Einsichten geliefert in die Grundlagen der Rezeptivität des Endometriums, speziell in die Zellbiologie des Uterusepithels während der Anheftung und Invasion des Trophoblasten. Sie ändern die traditionelle Sicht des Implantationsvorgangs insofern als sie zeigen, dass das Wirtsgewebe nicht nur eine passive Rolle spielt (Apoptose und Lyse) sondern von Anbeginn an durch zelluläre Reaktionen aktiv an der Herstellung des Kontakts mit dem Trophoblasten mitwirkt und den Implantationsvorgang von seiner Seite her kontrolliert.
