Forschungsbereich
HIV
Schwerpunkte in der klinischen HIV-Forschung der HPSTD-Ambulanz (HIV/AIDS, Proktologie, Geschlechtskrankheiten (STD)) sind das Altern mit HIV, chronische Inflammation, Komorbiditäten und Langzeittoxizitäten der antiretroviralen Therapie (ART) in Menschen, die mit einer HIV-Infektion leben, Weiterentwicklungen der ART, Heilungsversuche bei HIV-Infizierten mit Indikation zur Knochenmarkstransplantation, schwer ART-Vorbehandelte mit Resistenzen und HIV-Infizierte mit antiretroviralem Therapieversagen. Neben kardiovaskulären Erkrankungen werden besonders HIV-assoziierte Neoplasien wie das Kaposi Sarkom und das Analkarzinom wissenschaftlich untersucht.
Unser Zentrum nimmt an nationalen und internationalen Studien teil, z. B. an der START Studie von der INSIGHT-Gruppe, der ClinSurv Kohorte des Robert-Koch-Instituts, über das DZIF an einer Studie zu neutralisierenden Antikörpern und an einer HIV-Inzidenz-Studie bei Männern, die Sex mit Männern haben (MSM), im Rahmen des US Military HIV Research Programms (USMHRP) zur Erhebung der Durchführbarkeit einer HIV-Impfstudie. Alle Fälle von SARS-CoV-2 Infektionen bei HIV-Infizierten werden prospektiv systematisch erfasst und Proben asserviert.
STI und Proktologie
Prävention, Screening, Diagnostik und Behandlung von sexuell übertragbaren Infektionen einschließlich Hepatitis A, B und C werden im Rahmen wissenschaftlicher Projekte erforscht.
Als eines der ersten Zentren in Europa haben wir am Universitätsklinikum Essen die High Resolution Anoscopy zur Früherkennung HPV-assoziierter Läsionen und das Analkarzinom-Screening bei Menschen, die mit einer HIV-Infektion leben, eingeführt. Die Behandlung von Feigwarzen und analen intraepitehlialen Neoplasien gehört ebenso wie die Teilnahme an Impfstudien gegen HPV zu den Forschungsschwerpunkten.

HIV HEART Aging
Mit der HIV HEART Aging Studie findet unter Leitung des Essener Zentrums seit mehr als 15 Jahren die größte prospektive deutsche Kohortenstudie zum Altern mit HIV-Infektion und zu kardiovaskulären Erkrankungen bei Menschen, die mit einer HIV-Infektion leben, im Ruhrgebiet statt (unterstützt von ViiV, MSD, Janssen, Gilead und BMBF).
TECAIN
Die TECAIN Studie untersucht multizentrisch, prospektiv, randomisiert die Effektivität einer topischen Behandlung mit Trichloressigsäure (T) im Vergleich zur lokalen elektrokaustischen Abtragung (EC) von histologisch gesicherten, analen, intraepithelialen Neoplasien (AIN) bei Menschen, die mit einer HIV-Infektion leben (gefördert durch das BMBF).
PrEP und Impfung
Verschiedene Studien untersuchen Personen, die eine Präexpositions-Prophylaxe gegen HIV bekommen, in Bezug auf Effektivität, STI-Screening, Inzidenz und Prävalenz von STI, Veränderungen im Risikoverhalten sowie Eignung und Bereitschaft zur Teilnahme an Impfstudien.
Low Level Virämie, virologisches Versagen und Resistenzen
Trotz auf der Basis von Resistenztestungen, Nebenwirkungsmanagement, Compliance-Erhebungen und Medikamentenspiegelmessungen optimierter antiretroviraler Therapie besteht bei einigen HIV-Infizierten nach Ausschluss eines virologischen Versagens eine Low-Level-Virämie, deren klinische und immunologische Auswirkungen wir wissenschaftlich untersuchen (unterstützt von Janssen).
Auftragsforschung
In der Auftragsforschung beteiligen wir uns für Menschen, die mit einer HIV-Infektion leben, an der Weiterentwicklung der antiretroviralen Therapie (Injectables, Long-acting Substanzen, neue Substanzklassen besonders für schwer Vorbehandelte mit Resistenzen) und an Studien zu Impfstoffen gegen HIV, HSV, HPV und anderen STI.
